The Physical Therapist plans, implements, and evaluates specialized physical therapy services for PACE participants, ensuring comprehensive assessments, treatment planning, and education to promote safety and independence among seniors. They work collaboratively with an interdisciplinary team to tailor care plans, provide in-home evaluations, recommend home modifications, and enhance overall patient functionality through targeted treatment programs. In addition to delivering high-quality clinical care, the therapist actively engages in quality improvement initiatives, documentation, and ongoing professional development to support a people-first environment and improve the quality of life for the elderly.

JOB SUMMARY
Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching participant, caregiver or other appropriate representatives/family to maintain participant support in the community.
