We are seeking a dedicated and experienced Population Health Nurse to join our team. The ideal candidate will play a crucial role in assessing community health needs, developing and implementing health promotion initiatives, and collaborating with interdisciplinary teams to enhance health outcomes.
Assess the health and healthcare needs of diverse populations within the community.
Identify at-risk groups and subpopulations that would benefit from targeted health promotion and disease prevention initiatives.
Develop and implement intervention plans to address identified health threats and evaluate their effectiveness.
Collaborate with healthcare providers, community organizations, and stakeholders to coordinate care and resources.
Utilize data analytics to monitor health trends and inform decision-making processes.
Provide health education and coaching to individuals and groups to promote wellness and chronic disease management.
Participate in quality improvement initiatives and contribute to policy development aimed at enhancing population health. Home Indeed
Current, active, and unrestricted Registered Nurse (RN) license in the state of Illinois.
Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing, Public Health, or related field preferred.
Minimum of 2 years of nursing experience, with at least 1 year in community or public health settings.
Certification in Population Health Nursing or Public Health Nursing is highly desirable.
Strong understanding of social determinants of health and health equity principles.
Excellent communication, organizational, and analytical skills.
Proficiency in electronic health records (EHR) and data analysis tools.
